Relapsing polychondritis symptom tracker for free and free to share.

Getting a diagnosis for a rare disease is overwhelming. For me, I needed to get a handle on the overwhelm in as many ways as possible.

One of the first things I did was create a symptom tracking book for myself. I wanted something I could bring to appointments, use as a reference, and easily look back through when symptoms changed or flared. Turns out, I’ve taken it to the ER more than once, too.

I'm giving this away as a free download because I feel ts such a valuable tool.

This download includes greyscale versions of my Relapsing Polychondritis symptom trackers. It is free to download and free to share widely. Print as many copies as you need, build a binder, or organize pages in whatever way works best for you.

If you’d prefer the convenience of a professionally printed bound book, the full color version is also available for purchase through Amazon or Lulu.
